Our Central Scheduling Department streamlines the insurance authorization process and patient scheduling for your convenience.

To refer a patient, please fax the following items to Central Scheduling:

• Medication prescription with ICD diagnosis code (orders must be renewed every six months).
• Patient demographics with insurance information.
• Associated lab results or an order to draw labs.
• Physician notes indicating the need for treatment.
